Family & Friend phone numbers
Do not rely on your cell phone to always have betteries or to always be in working order. I know many people that don’t know their relatives phone numbers because their cell phone stores it for them. In a survival situation you may need to call a family member or friend from a pay phone or a rescuer may need their number. You should put together a list of 5-10 numbers you would most likely call in case of an emergency on a piece of paper and have it laminated.
For survival purposes it may be a good idea to have a couple of these lists. One for your wallet, your car, and your home so that you are never far from one.
I’m sure you could also think of other items to note on the papers’ blank space as well 
(Addresses, directions, how to make drinkable water, fire starting tips, etc)
